As an Archway Clinical Program Manager:
You will be responsible for establishing relationships with our provider customers which include specialty physician groups, hospitals and health systems.
You will help our provider partners deliver increased value by improving quality and outcomes for patients while managing the costs of care.

Key Functions / Responsibilities
Develop strong relationships throughout customer sites including Senior Leadership, the Discharge Planning and Case Management teams, Nursing, Physicians, and key post-acute providers – Physician Groups, Skilled Nursing Facilities (SNFs), Home Health Agencies (HHAs), etc.
Identify and document specific program goals including the high priority care management care redesign efforts required to improve performance in the program’s key areas: SNF utilization, SNF Length of Stay (LOS), Readmission Reduction
Work collaboratively with the Customer team to integrate the Archway Health Model into the current workflow and care management activities at Customer sites
Actively participate on the program’s performance and ongoing quality improvement
Work with program partner to develop formal metrics and goals of program performance including quality of care, efficiency and patient satisfaction measures
Conduct and coordinate formal and informal program training sessions.
Assess, report on and communicate program status on a periodic basis to all program stakeholders.
Collaborate with and motivate clinicians and cross-functional partners around key clinical initiatives and drive for results and effective execution;
Credibly interface with senior executives, including the ability to communicate with non-management stakeholders effectively;
Active Registered Nurse licensed in the state of Florida required
Case Management certification by the Commission for Case Manager Certification (CCMC) or American Nurses Credentialing Center (ANCC) preferred
Minimum 2 years working in case management or care coordination
Bachelor’s degree or equivalent required
50% Travel within Brevard County, Florida Required
Demonstrated experience in leading and managing complex projects that are strategic in nature and scope.
Several years of experience leading care transitions from hospital to home across multiple settings along the care continuum;
Experience with performance measurement, outcomes analysis, risk management, clinical data, and monitoring tools
