Archway Clinical Program Manager

Overview

As an Archway Clinical Program Manager:

You will be responsible for establishing relationships with our provider customers which include specialty physician groups, hospitals and health systems. 

You will help our provider partners deliver increased value by improving quality and outcomes for patients while managing the costs of care.

Key Functions / Responsibilities

  • Develop strong relationships throughout customer sites including Senior Leadership, the Discharge Planning and Case Management teams, Nursing, Physicians, and key post-acute providers – Physician Groups, Skilled Nursing Facilities (SNFs), Home Health Agencies (HHAs), etc.

  • Identify and document specific program goals including the high priority care management care redesign efforts required to improve performance in the program’s key areas: SNF utilization, SNF Length of Stay (LOS), Readmission Reduction

  • Work collaboratively with the Customer team to integrate the Archway Health Model into the current workflow and care management activities at Customer sites

  • Actively participate on the program’s performance and ongoing quality improvement 

  • Work with program partner to develop formal metrics and goals of program performance including quality of care, efficiency and patient satisfaction measures

  • Conduct and coordinate formal and informal program training sessions.

  • Assess, report on and communicate program status on a periodic basis to all program stakeholders.

  • Collaborate with and motivate clinicians and cross-functional partners around key clinical initiatives and drive for results and effective execution;

  • Credibly interface with senior executives, including the ability to communicate with non-management stakeholders effectively;

Qualifications

  • Active Registered Nurse licensed in the state of Florida required

  • Case Management certification by the Commission for Case Manager Certification (CCMC) or American Nurses Credentialing Center (ANCC) preferred

  • Minimum 2 years working in case management or care coordination

  • Bachelor’s degree or equivalent required

  • 50% Travel within Brevard County, Florida Required

  • Demonstrated experience in leading and managing complex projects that are strategic in nature and scope.

  • Several years of experience leading care transitions from hospital to home across multiple settings along the care continuum;

  • Experience with performance measurement, outcomes analysis, risk management, clinical data, and monitoring tools

Nurse Care Manager

You will strive to empower members with the knowledge they need to make informed decisions about their care, remove barriers to good care, monitor progress, and address any concerns as they arise.

You will also work with the Archway team using data, market knowledge, relationships and other tools to develop and nurture partnerships with high performing specialists in all categories and multiple markets across the country.

Key Functions / Responsibilities

  • Be eager to do the best for the members we serve, treating them as you would your family and friends

  • Be comfortable working in an entrepreneurial environment where processes and procedures are rapidly evolving

  • Be excited to get hands on with manual research, clinical and non-clinical content development, etc. in addition to owning the relationship with the member

  • Be resourceful in addressing the member’s needs, using all of your expertise, relationships, and creativity to provide an excellent member experience

  • Interact with members via phone and live-chat technology

  • Use motivational interviewing techniques to get to know the member and build trust, while capturing information about the member that we can use to better serve them

  • Translate complex clinical information into language an average person can understand

  • Validate proprietary and public physician performance data to identify high-value specialists best suited to meet the needs of the member via various research methods, clinical training and networking

  • Based on your understanding of the member’s needs, match the member with a high value specialist

  • Compile physician recommendation information and relevant clinical information and deliver to the member verbally and through available technology tools

  • Remove barriers for the member, including scheduling appointments on behalf of the member

  • Engage with the member throughout their care journey, providing relevant guidance and curated clinical content at appropriate points in their journey

  • Coordinate care, monitor the member’s progress, and advocate on behalf of the member, coordinating care with the member’s providers as necessary

  • Collaborate with the Archway team to constantly improve the service we offer

  • Be comfortable using a collection of technology platforms to deliver exceptional service to members, and be open to doing things manually when enabling technology is not available

Qualifications

  • A passion for improving healthcare for patients and consumers by proactively helping them get the information and access they need to make the best decisions for them and their family

  • Active Registered Nurse licensed in the state of Massachusetts required

  • Case Management certification by the Commission for Case Manager Certification (CCMC) or American Nurses Credentialing Center (ANCC) preferred

  • Minimum 2 years working in case management or care coordination, with a focus on a pediatric through adult population

  • Bachelor’s degree or equivalent required

  • Experience working with technology and software tools to deliver care coordination services

Value Based Care Account Manager

Archway Health is seeking to augment its account manager teams.

Archway Health’s mission is to fix healthcare through payment reform.  Archway is a national leader in developing and implementing value-based care programs in partnership with providers, employers and payers across the country.  We do this through a mix of great people who are passionate about improving healthcare, sophisticated data analytics tools that drive our unique insights, and easy to use software designed to help patients get better as quickly and safely as possible.   We work with specialty providers in all types of clinical areas – oncology, orthopedics, cardiology, pulmonology, obstetrics, hospital care, and post-acute care – all of whom are experiencing real success with improving outcomes and the patient experience while also dramatically reducing healthcare costs.

This role will:

  • Develop and lead principal relationships with our provider customers, which include specialty physician groups, hospitals & health systems;

  • Help our provider partners deliver increased value by improving quality and outcomes while reducing the cost of care;

  • Onboard customers onto Archway’s tools and services platform;

  • Provide excellent customer services and support for our customers;

  • Solve customer problems and create value both independently and in collaboration with the full Archway team;

  • Work with Archway’s data analytics and patient tracking software tools to monitor progress and improve the care delivery process;

  • Identify opportunities to meaningfully improve processes, quality, outcomes and reduce costs;

  • Demonstrate measurable and consistent results in improving value;

  • Interact directly with administrative and clinical leaders within our customer organizations;

  • Help to grow our business by building great customer relationships and helping to attract new customers;

  • Work within the Archway Account Management, Analytics, and Technology teams to continuously improve our tools and services;

  • Be a good team player and become an integral member of the Archway team and the Value Based Payment teams of our customers.

Qualifications:

  • 2-3 years experience working with healthcare providers or payers;

  • A passion for improving healthcare for patients, providers, & purchasers;

  • Experience working with technology and software tools, including Excel, PowerPoint, Tableau and social media

  • A successful account management background

  • Strong communication, presentation and organizational skills with the ability to work independently

  • A great team player

  • BA/BS degree

  • Willingness to travel

Physical Requirements

Position requires the ability to travel up to 25% of the time as well as work under normal office conditions where the individual will walk, stand, stoop, sit, manipulate / adjust items, and keyboard on a regular basis. Must be able to analyze / synthesize data and communicate issues, findings, processes, solutions and ideas in a clear, concise, technically-proficient and consultative manner.
